Factors Affecting Prolonged Length of Stay of Patients with Psychiatric Disorders in Acute Hospital

Abstract

|
Objective: To investigate characteristics of inpatients with complex diseases, which reflect psychiatric disorders and chronic diseases for the acute hospitals, and to examine the factors affecting prolonged LOS(length of stay) in-patients with psychiatric disorders.
Method: Data was selected from the KNHDIIS(Korean National Hospital Discharge In-depth Injury Survey) database from 2015 to 2017 by the Korea Centers for Disease Control and Prevention. The subjects were in-patients over 19 years old who had psychiatric disorders disease code. The factors affecting hospital stays over 90 days and under 90 days were divided into patient characteristics, admission and disease characteristics, and hospital characteristics. Chi-square test was conducted whether there is a significant difference between over 90 days and under 90 days according to each characteristic. Also, logistic regression was performed to affect factors on over 90 days.
Result: When the result of discharge is death or other hospital transfer and then going home, the higher the CCI(Charlson comorbidity index) score, two different psychiatric disorders than single psychiatric disorders, medical aid, emergency rooms, and the metropolitan city had prolonged LOS. But, the younger age, the complex between psychiatric disorders and chronic diseases, metropolitan had not prolonged LOS.
Conclusion: In developing strategies for improving the financial adequacy of patients with psychiatric disorders, management strategy of LOS that reflects the variety of complex diseases and the severity of diseases will need to be set.
